(ON Only) Patient Consent

Prior to completing a medication review, pharmacists must now collect patient consent through the standardized Patient Acknowledgment of Professional Pharmacy Service form. This is an annual mandatory process for the patient to acknowledge professional pharmacy services. This is facilitated with the use of a mandatory form and when completed by the patient essentially confirms the patient’s understanding of the MedsCheck service.

This form, which is built into the Medication Review App:

  • Must be completed annually and provided to the patient; a completed copy is maintained at the pharmacy.
  • Must be done before the pharmacist conducts the MedsCheck service and before the pharmacist bills the ministry for the MedsCheck service through the Health Network System.
  • Replaces the patient’s signature on the MedsCheck Personal Medication Record

Once patient consent is collected, a copy of the signed form is saved to the Medication Review History tab.

Note: Patient Consent must be collected once every 365 days. If the patient already has consent on file, your pharmacy will not be prompted to collect consent until 365 days have passed.
